Change it whenever someone with access leaves, and on a regular schedule beyond that. Codes get shared, watched over a shoulder and written down, so a code that has not changed in two years is not really controlling much. Systems that support individual codes per person make this far less disruptive.
On many systems, yes. Individual codes mean you can remove one person's access without changing anything for anyone else, and depending on the unit you may also get a record of which code was used and when. Standalone units vary in how many codes they hold, which is one of the things we check before recommending one.
Most units have a recovery route through a master code, a programming key or a reset procedure, and there is normally a mechanical override on the door as well. If you have inherited a keypad and nobody knows the master code, we can usually get it back under control without replacing the hardware.
It depends on the door, the unit, whether it is standalone or wired into a wider system, and the power and wiring involved. A single standalone keypad on an interior door is a very different job to a keypad tied into a networked system. We quote per door after a free site audit, with clear pricing and no hidden fees.
